Insurance
Our office is committed to helping you with your insurance. We will work with you to maximize your insurance benefits. Due to the complexities of insurance contracts, we cannot guarantee your coverage and are only able to estimate what payment your insurance may make. We are HIPAA compliant and therefore regulated by the laws governing providers. Due to these regulations, we may not be able to address some of the questions you have for your insurance company. We will handle as much of your insurance needs and questions as possible.
Please always bring your CURRENT insurance cards with you and be prepared to show them if we ask. If they are different than what we have on file or it is your first visit, we will make copies of them for our records. We are only able to bill the insurance you provide to us. For established patients, if your have any changes to your address, telephone number, or work situation, please inform us when you arrive at the office for your visit.
We accept most insurances and participate with a majority of companies. These include: Medicare and Most Medicare products, Blue Cross/Blue Shield, United Healthcare, Aetna, Cigna, MVP, GHI, and Tricare.
If you do not see the name of your insurance company listed, please ask when making your appointment. As a specialty practice we are considered medical providers, caring for your medical concerns, and our services are not usually covered by your straight vision plan. Again, if you have questions concerning this matter, please do not hesitate to call.
If you have an insurance company that requires a referral/authorization before seeing a specialist, please obtain this from your primary care physician before coming to our office. If you already have a referral on file, be sure you have visits remaining or that it has not expired. We will assist you in this area as needed.
Many insurance policies now have a copay due at the time of your visit. We are required by contract with your insurance to collect your copay when you arrive for your appointment. For your convenience, we accept cash, checks, money orders or Visa/MC as payment.
